So, I'm putting together my first post-41F Form 4 on a Trust paperwork. There are only two trustees (me and the Mrs). I figured I'd bounce it off the forum to confirm we're getting all the ducks in a row.

To the ATF

  • Form 4 (2 copies)
  • Copy of Trust (1 copy)
  • Responsible Persons form for me with photograph (1 copy)
  • Responsible Persons form for my wife with photograph (1 copy)
  • Completed fingerprint cards for me. (2 cards)
  • Completed fingerprint cards for the wife. (2 cards)
  • Check in the amount of $200.00.
To the CLEO (Sheriff)

  • Form 4 (1 copy)
  • Responsible Persons form for me, sans photograph (1 copy)
  • Responsible Persons form for my wife, sans photograph (1 copy)

Based on what I see posted on the ATF website, it would appear that you have them all correct. Each trustee needs their own responsible person form and photo/fingerprints as you have outlined.

Thank you. Seems everybody who knows you got the stuff wants to borrow, shoot and have fun with the stuff, but nobody wants to be on the Trust. Is this a common problem? Nobody wants to sign up. :(

I really don't want anyone other than my son on my trust. He's the only other person I'd let borrow my stuff. Anyone else who wants to shoot with me, and brings their own ammo, is welcome. The stuff never leaves my control.
