This stuff is so useful in many aspects of everyday life. I use it for fish cleaning/cutting boards.
This material is also what NW Safe used to effortlessly glide my huge Colonial 50 safe into "the Room". It was slicker than owl snot on a rainy night!!! I'm hoping to recreate that experience and take that big MOFO south to the Ponderosa and this will valuably aid me in my exodus. :s0155:
I've got a table saw to cut it up.

TAP plastics sells it. 4x8 sheets or smaller. They'll custom cut stuff for you too. There's several locations around the greater PDC area.

IMO, it's not priced too bad either.
 
See if you can rent an air sled. Way easier than sliding it!

Are you going over carpet or a hard surface of some type? While I have used HDPE to slide when necessary, I've also used oak dowels cut to length to roll things on. If it'll work, the air sled is by far the easiest option. I've moved fully loaded 42" wide Sub Zero refrigerators with them by myself and it went super easy. Hopefully you can find one locally that'll work on a thousand pound safe.

It's gonna be going over hard surfaces and carpet and I want something I can use for both. I've decided to go with 1/4" HDPE smooth sheet product that I can cut into 1" strips, glue and screw it to a 1 1/2" x 1 1/2" x 36" board.

I have dealt with TAP plastics for everything from plastic bottles and window tint to the $700.00 of scratch resistant Polycarbonate windows for my Willys jeep hardtop.

The also have a scrap box with smaller pieces discounted

I have always been happy with my dealings with them.
