Here's one that I've never seen before. I was shooting three boxes of new Winchester 380 Auto , 95gr FMJ "Target" ammo. Somewhere in the second box, one round did not fire. I could see that the pistol did not go into battery. I ejected it and put it back in the magazine and tried again with the same result. There had not been any cycling issues before or after that. There was also one dud. It turns out that the brass on the round that did not cycle was way too long. I measured 18.3mm. A normal spent round is very close to 17mm. Picture attached.

That looks like a QC issue. I am highly curious how a long case got though much of the process like that, starting with the case trimming step. We will probably never know.
