Here is one I am actually responsible for: 5 rounds 38 spl swc. Gun is now a 4 inch and shoots just fine. I got lucky!

I don't have any personal pictures of squib loads but I'll share crazy ones I've seen on the net.

These actually look more like metal fatigue/ over powered loads.

OK heres my story. I bought a used Bryco 59 (I here the OMGs already) 9mm at a local gun show. This was purchased as a "truck gun" at a good price. My son and I shot this weapon at the range and put about two clips thru it (25/26 rounds). Reloaded the clips and on the second shot the slide blew away from the frame and into the air. We were shooting hand loads. Lesson learned.
