Direct mill everything. Personally I hate optic plates.

I like the idea of the Aimpoint Acro-P2, Steiner MPS, Holosun 509T, ect. mounting system where it's a horizontal bolt retaining the optic (picatinny style) vs. the vertical bolts that go directly into the slide.
 
Ive never heard anything bad about S&W quality, i doubt its the screws. I think the plastic plate "rocking" under recoil is a valid theory ( which would be on S&W shoulders...)

Screws are mass produced, small chance is could be the metalurgy but I doubt it.

I just hope for your sake they dont need to replace the whole slide over this...

For my custom 10mm glock longslide, one screw has to be shorter and I didnt like how short, i orderd the next fractional size from mcmaster carr. It was a harder to find in-between size that added another thread without interfering... i might have gave it a slight file on the tip... No plastic spacer, several hundred rounds no and no issues.
I had JagerWerks mill one of my G19 slides. I had the same problem you have. I needed to use two different depth mounting screws.

Yeah, i dont like how short the one is. I have a custom G19 slide and a custom G40 slide (10mm) and for both i made custom screws for that weak side. Gains at least 1 more thread, maybe 2. Worth it for my OCD... :p One I went to Ace hdwr and carefully filed down to max size (chamferring the thread tip under a magnifyer was interesting...), the other i ordered that in-between size. I cant recall which gun has which now.... :p

Did you notice my 180 handloads clock at 1,250+. BIG difference in recoil!

As you know, your underwood's are extreme! Much recoil!

I sent the gun to S&W for warranty repair, via the label they emailed me. My tracking number says Fed-Ex delivered it to them a week ago.
Radio silence. Not a word from S&W. I am getting a bit worried. Is this normal, to be ignored as such? Anyone have experience with S&W?
It seems to me they could have at least acknowledged receipt of my pistol.

Its not uncommon to have to reach out to them for an update, but Id wait till late next week due to the holiday.
 
I recently picked up a plate from Leupold to run their DDP on my M&P 1.0 CORE. That plate is all metal goodness and is THiCC with two C's! The DPP is a really tall optic and this only adds to the overall height. No big deal, as this gun is strictly a range gun.

I'll try and get some pics and post them up.

So, today marks the date.
I sent my Smith to the factory to have the broken screws removed, TWO MONTHS ago!
I emailed them again yesterday and still have no response.
Asked them if they lost my gun and if I should fill out a stolen weapon report.

Radio silence - this is what to expect from the people at Smith & Wesson.
Can customer service get much worse?

Yes, Ive got worse but hoping for the best for you but I suspect they are scratching their heads trying to figure out how to extract those screws or making you a new slide.
 
In summary. After months of waiting, S&W simply sent me a new slide.
They did not bother to try and remove the broken screws.
This could have been taken care of in a week.
