The blue box seems pretty clear, so I don't think you need to fill it out. The other thing to keep in mind beyond the insurance is that any accident that shows up on your CarFax will significantly hit you when you try to sell it or trade it in. The buyer has no idea how bad that accident was.

My crash was quite clear we both had to file a DMV report. Trooper mentioned such & not to put it off / forget.

Severe damage & 1 vehicle required tow/not drivable. Did so overnight. Pretty easy, plus uploaded copies of the OSP report.

Didn't think to take pics of her car. Should have, just because. Given she's at fault, admitted to fault to the trooper & was cited for such, doesn't really matter what lawyers want to try to do…

OP's circumstance, no, no DMV report required.

Minimal sore back/tension for a bit yesterday eve. Like I get with "life stressors". Not nerve/disc/structural.

Did some easy stretching & de-stressing relaxation at work & no problems overnight. Helped that there weren't any complex issues too...
