It seems like you are likening CHL holders to soldiers. Or police. Just because one has a CHL doesn't mean that person is legally required to intervene. You have to use your own judgement.

I am not risking my health to intervene when some dope fiend winds up with no pulse and no respirations. I leave them. I don't lay a hand on them. Many times they have diseases. They have no right to my medical expertise. I will call 911 and give their location, and then I hang up and I split. I use my own judgement, and when it isn't worth it, I walk away.

No permission from the government necessary in Montana, unless you want to get a permit so you can travel to states with reciprocity. In the past I've taken both the Oregon and the Utah classes. Training is an ongoing commitment. I used to train. Training should include things like weapon retention, when one is allowed to act, whether one is legally obligated to act, and improvement and retention of skills.
