I just got a 12 ga. H&R.
It has no model number on it.
It says re-enforced breech on the side.
Serial is A4924X
The receiver looks like nickel steel, the barrel is blue.
The year is not visible.
I love me some H&R single shots! I have a .410 that was my grandfather's, a .45-70 Buffalo Classic and a 12 guage full choke that I picked up at a gun show in October for $80. Good find!

Post it over on Greybeards outdoors, in the H&R shotgun section. Someone over there should have the information on it. Couple of folks who hang out over there, have written books about H&R.
