I believe your first answer is correct. When they were still kids their mom was quoted as saying that if one sister had a runny nose the other sister could take cold medication and it would work for the afflicted sister.

What if Brittany one day became tired of having Josh around and MURDERED him with her half of their body!

Each twin is considered an individual regardless of being conjoined, so Brittany could be charged with a crime on her own account. Would Abby be forced to go along for the ride to prison, or would Brittany get a pass because punishing her would unjustly also punish Abby? It boggles the mind!!!! o_O
 
Be pretty hard not to get a conviction on at least co-conspirator after the fact. Unless Abby was willing to immediately try to call the cops. But then maybe Brittany could fight to stop her?

But that brings up other questions, like did they ever actually fight as kids? Considering how they are laid out it would be pretty hard to block an incoming punch or a kick...
 
What if one has an aneurysm and dies? Will the other sisters body keep the dead one's organs from decaying due to fresh oxygenated blood? What about heart/lung failure? What if one had heart failure and the other had lung failure? What if one had really bad breath and wouldn't stop talking to the other? What about major surgery? Do they both need insurance or do they get to buy a single policy?

I've briefly scanned articles (since the original story came out) that they are all surprised and dismayed by all the speculative attention they've received. But hey, what'd they expect by going public in the first place?
Yeah, they kinda did that to themselves with the TV show and everything. Regular celebrity marriages are a spectacle (whether they want it or not), how did they think their unique circumstance would not draw all the attention too?

There have been lots of other people who have had unique and interesting medical conditions. Quite a few of them figured out that the way to not get attention was to go completely dark; no media, no interviews, no comments to the editor, nothing. Any public engagement brought a disproportionate amount of attention simply due to public curiosity. Getting on a TV show, and now announcing a wedding publicly, is not a great way to lay low. People are curious, and absent actual answers will speculate wildly on a topic of interest. This is a hole they dug themselves.
