This IWB hybrid holster (kydex shell with leather back) is from Concealment Solutions. It is for the Glock 26/27/31 series, all Gens. I put an RMR on my G27 and no longer need this holster. It is tuckable. I made the clip give the holster a very slight forward cant (maybe 5-7 degrees?) so it can be used appendix to hip carry.
It is in great condition and I only used it a dozen or fewer times.
