I ended up using Lakota's solution. Rolled our own on physical cards, scanned to an image, then uploaded to the aforementioned. Conversion was done in a few minutes. I've used the two files it produced twice now; one was a Form 4 (silencer) that was approved, the other a Form 1 (short-barrel shotgun) that is pending. (Though eForms accepted the EFT files and indicated the format is what it needed.)

How did they handle getting your personal info, like your SS number?

If you mean the data at the top of the FBI cards (e.g., name, DOB, race, etc.) that is entered on a webform and then it is encoded in the resulting EFT file.
