It's a firearms disposition form. It's to move the "non-registered" gun from your record into that of the new owner.

I once went into the Clark County Sheriff's office to renew my CCW and the friendly lately asked me if I still owned all of these handguns? At the time the list was 25-50 guns, if I had sold any I could fill out the form and remove them from my name.

It's a "Voluntary" form a seller sends to the state to show he sold the pistol....and to "who" it was sold to.
