I actually had a .40 run through a .45 once, buddy loaded the mag and handed it to me. None of us owned a .40 so its still a mystery where it came from, luckily no one got hurt and the firearm survived without a problem.

I purchased a case of 270 Win manufactured by Remington, I pulled a few boxes out and put them in my pack, on my way to go coyote hunting.

I got all setup and loaded my rifle, right from the factory packed ammunition box. After a bit a coyote slipped out and I knocked it down. I tried to chamber the next round and I could not close the bolt. I opened the action and looked at the round and it was a Remington 30-06 round, that was in the box of 270 Win.

Since that day, I always double check each round before I load it in a magazine or firearm.

Anything mass produced can have flaws or be improperly packaged.
