ATF Form 4 question

When submitting a form 4 to the ATF, who is responsible for affixing the 2"x2" passport sized photo to the form.
Do I glue it on, or is that done at the ATF office.
I don't want to screw this up and have a delay on this paperwork.

it says it quite clearly to me on the form a couple of times who's responsibility it is. i've used a glue stick for 20 yrs with no kick back

I did. And I just used a glue stick.
Go over everything a few times before sending it. I wasted an extra month because I forgot to sign one of the lines and had part of it kicked back to me.

I have used glue and staples. No issues with either.
