Not counting the standard Mil-Spec FCG, have you ever had a drop in trigger assembly fail on you? If so, what part failed and how would you improve your next trigger decision?

I.E.

 
The only failures I've had with drop in cassette style was from adjusting them as light as they would go and then not hitting the mil spec primers hard enough to ignite them. User error on my end.
 
Early on, as in several years ago they could be finicky, but the current state is pretty solid. I have several under $130 brands with no regrets.

I would love to try some more expensive brands to see if they are worth the extra. I like the POF 4lb trigger. Noticeably heavier than some but I prefer the feel to my others. More or less the 3.5lb from CMC, PSA and Rise feel about the same to me.
 
I had a CMC cassette trigger fail, I think it was failing to reset but no clear memory. It might have been a dead trigger. I called, they said send it back and we will fix or replace. They did so quickly, and many 1,000s of rounds have since been fine.

I had the fancy Rise Armament Iconic cassette trigger fail to reset, it was pretty new. They sent me a new one same day I called.
