Check the rifles headspace. Happens with hot loads in a rifle or excessive headspace.

In the case of a rimmed cartridge, like 357, headspace is the the case head rim clearance with the bolt closed. It should be between 0 and 0.021" for new ammunition. If you run reloads, you want it tighter, as close to zero as possible.
