My buddy called after inspecting the brake closely and said the bullet was striking the brake. He is checking the brake fully now to see what we can learn as to WHY it was striking. I will update when I have more info.
 
I am sure it's the brake. Your getting light strikes against it. More then likely the muzzle wasn't threaded correctly or the brake wasn't threaded correctly. I've seen this at least 5 times. Even big manufacturers have had breaks threaded incorrectly.
