My first week "In Country" I was called on to Investigate a 106mm Recoiless Rifle accident. A kid(17 maybe 16) had crawled up on the back of the Jeep, grabbing the Cocking Handle, Shutting the Breach, Triggering the Round and taking his own head off at the Shoulders. His parents didn't even know he was in the Service! Not cool but that's what can happen when the PM isn't being performed when it should have been.
