Aye. Looks exactly like that's what happened.

did a 180 in the receiver on ejection.....then bolt closed. gonna be difficult to reload that one..................

It did eject but landed off to the left of me somehow. All the others were flying right a good 10 feet. I'll get the needle nose out and fix it

Could be dirty chamber or dirty case or bad ejector extractor combination but the case didn't fly out as it was supposed to and got bent in the action.
