Some of us have.I think surgeons wear masks both to protect the patient and themselves... sneezing or coughing droplets into an open wound isn't really a good idea; they try to keep everything going into the body cavity pretty sterile.
They use N95 or further measures for instance if they are working on an airborne pathogen such as pulmonary TB. Stabdard cloth masks only reduce the viral load and droplet transmission, they don't eliminate an airborne virus.
