Yes, it has happened more times than many would imagine.
In the 2nd generation of the AR, Colt went to a smaller/lighter firing pin to lessen the danger, but it's still not unheard of,
In recent times it has most often happened when people re-load ammo with non-recommended primers, such as the copper/gold colored primers made by Winchester,, the silver colored ones have thicker cups.
The Federal 205/205M, the CCI 450, and the CCI #41 are the best primers for AR use, their are a couple of others that I don't recall the names of.
Their are many YouTube vids about slam-fires in ARs,, a friend of mine had it happen to him.
The best answer is a Titanium firing pin,, and when you re-load for an AR make d**m sure the primer is seated below flush.
