Pick it up in Iowa, then drive/fly home with it. Fill out a .20 with the permanent address change and your done. Your not applying to transport it, theres a absolutely no need, your just letting them know you moved.
Or as others have said, cancel your Form 4. Have the dealer transfer it to a local dealer and resubmit the form 4. You'll likely end up paying more fees, add even more time waiting, and pissoff the dealer in Iowa, but whatever.
