A birth certificate or passport is required along with a new application. The fee on the form here in Benton County Oregon is $50 but she said I had to pay $75 because the state raised the fee. I asked what the state had to do with it and was told that they get $10 if they have to check fingerprints which makes absolutely no sense to me. She told me she did about 12 applications or renewals a day. I had to wait almost three months for an appointment and the process took a whole five minutes. I bit my tongue and left.
