Or it could be a case of dropping in a 16ga (barrel obstruction) and then following it with a 12ga....but you're probably right.
...or chambering a longer shell than its rated for. (and yes it is possible to do this, example; the length of a 3" shell is the length after firing, and is around 2.75" before firing, so you can actually chamber it into a 2.75" chamber, with dire consequences)
