Gilmore Group/CDC "Health Survey"

Got a letter from WA Dept of Health (Mary C Selecky) telling me that the CDC has contracted the Gilmore Group (does phone surveys) in Seattle to gather info to help "target state programs and activities...for a safer and healthier Washington."
Has anyone replied to one of these phone surveys, what kind of questions do they ask, what do they mean by "activities" and "safer"?

I was contacted, and yes they did ask who I was Blah blah, Then later if there were any guns in the home, after they asked about "recreational activitys".
