Clearly he had not learned to keep finger out of trigger guard until you're getting ready to fire.

I have to admit that I just dont trust holsters unless they have a retention strap. And these days, even some of those, the snap actually comes undone if you give the gun a sharp tug of the sort that the gun might experience if you fell down, or an attack started by a punch or push that through you several yards.
