    I am pretty sure that if any health care provider ever handed me a questionaire or actually asked a question about my owning firearms I would just hand it back or explain I do not discuss Religon with others and would appreciate if they did not attempt to discuss it with me.

    works on the people who knock on my door peddling God in what ever form. Should work as well on Doctors

    Man, the crowd in oly, from the gov on down, would jump at the chance to force this down our throats. I like Mark's answer but honestly doubt I'd be as polite. =';'=
