For those interested or curious, here's what I use for a bound book template.
I use a legal size paper in a ledger style binder with the acquisition on the left half, and the corresponding disposition on the right half printed double sided.
I print, then 2 hole punch, and voila. The margins are SUPER tight to the edge of the paper and may need to be adjusted for your printer.
I prefer the legal size of 8.5" x 14" - makes it easier to hand write items in.
